现将《201*至201*学年度普通高中学业水平考试工作计划安排》印发给你们,请据此安排好各项工作。如因故变更,另行通知。

考试工作计划安排
一、时间安排
时间9月30日前10月31日前11月1日至10日11月3日至5日工作内容下发新生建档材料;各地新生注册,收缴考籍管理、水平考试合格审定费。各地完成新生注册,上交考籍管理、水平考试合格审定费。组织201*级至201*级学业水平考试报名工作召开7月份学业水平考试工作总结会和12月份学业水平考试考务会。11月10日至11月25日整理考籍,编排打印考场,上报具体考点地址、试题数和巡视员名单(2名)。11月14日至20日组织命题11月28至12月18日印制试题,录制磁带。12月19日至22日12月21日12月21日至22日各市调试考点监控系统。各地领取试题,其中青岛、枣庄、烟台、威海、日照、临沂和菏泽于20日下午到达济南。21日下午召开省派巡视员会议,22日省派巡视员到达巡视地。201*级至201*级学生学业水平文化课考试:12月23日至25日23日8:00-9:30地理,10:15-11:45历史,14:00-15:30物理;24日8:00-9:30生物,10:15-11:45化学,14:00-15:30政治;25日8:00-9:30数学,10:15-11:45外语,14:00-16:00语文。12月26日至27日12月28日12月29日至201*年1月15日阅卷2月8日前3月底以前4月1日至30日5月7日至25日5月4日至9日5月20日前全省学业水平信息技术学科考试上报信息技术考试作弊学生名单12月29日至1月15日各地组织阅主观卷。2月4日,审定成绩,考生成绩上传平台。各学校完成本学期在校年级学生发展报告的相关工作,并上传平台学校上传201*级第六学期学生发展报告相关内容。打印、核发201*级学业水平考试合格证,处理201*级学业水平考试合格证的遗留问题各学校组织201*级学生学业水平考试报名工作学业水平考试考务会;组织命题。

(一)报名考试及成绩公布程序
201*年12月份学业水平考试201*级只允许报名参加语文、数学、外语3科考试,201*年6月份学业水平考试仅允许201*级学生参加。学校登录山东基础教育信息化管理平台学业水平考试模块进行考试报名,各市教育局用修改后的原展创程序下载报名信息,进行编场、阅卷、上报成绩按照原操作程序进行。成绩经我处审核后直接上传平台,不再单独下发,各市、县(市、区)、学校可通过平台下载。
(二)各地可根据当地实际一年内安排几次学生信息勘误工作,201*级最后一次的信息勘误必须由每个学生确认后签字,签字表学校保存备查。高中毕业年级下学期及毕业以后不再办理因身份证号或其他学生身份信息错误造成的合格证换发业务。
(三)201*年5月7日至25日,处理201*级学业水平考试合格证的遗留问题。各市在此期间必须严格认真核对毕业学生的情况,毕业证必须在高考前发到学生手中。
(四)外省转入学生需提供原学校、市、省级转学手续、成绩证明和学生信息。学业水平(或会考)成绩证明,须经我处审核确认。

本试卷共三部分,共15页(试题卷12页,答题卷3页)。满分135分,考试用时120分钟。注意事项:
